Cleanup:
 - remove struct kmembuckets (dead)
 - correctly deadify MALLOC_XX
 - remove MALLOC_DEFINE_LIMIT and MALLOC_JUSTDEFINE_LIMIT (dead)
 - remove malloc_roundup(), malloc_type_setlimit(), MALLOC_DEFINE_LIMIT()
   and MALLOC_JUSTDEFINE_LIMIT() from man 9 malloc
