Yes, this may be the problem.

Unfortunately I have no VAX online right now. Most likely you need to
boot the instalation system, drop to a shell, copy /usr/mdec/boot to
/boot and use installboot(8) to install the first stage boot block in
the raw file system. You can get the man-page online:
