Use NetBSD-current, not 1.4.1.
Yes, there where some odd problems with 16MB and later 32MB. But this is
gone. The hardware of the VAXstations, especially the SCSI adapter, is
much better supported in -current. Get the latest snapshot from
ftp.netbsd.org.
