I now have the hardware dma translation map (sgmap) working on the vs4000
workstations.  I have modified the vsbus lance attachment to work with it
(as well with older VS31000/VS2000).  (that's the buf 0x0-0xffff).  This
is a prerequisite for getting SCSI support working.

The biggest problem is that interrupt probe for the VS4000 assumed that
the lance has a valid pointer to an init block.  but since the sgmap was
nuked, there wasn't one.  So now the probe routine provides one.
