When I installed 12C on a VAXstation 3200 (with tk50) , I had problems
with 12C's file1, too. 
I think I got the installation to work by using 12`s file1 instead, and
additionally using 1.2-distribution's /boot and /usr/mdec/* files to make
it boot after installation.

You may hope this applies for a 4000, too.
