I only have 6.5.0 foundation CDs and thus was not able to install Irix
on the O2 (due to the RM5200 cpu from what I know). When booting the
6.5.17 overlays miniroot I could access the harddisk and read/write from
it.
My SCSI chain consists of one (or two) SCSI disks which are detected
from "hinv -t". I removed the CD-ROM as it seems to be broken (I cannot
boot from it). The SCSI terminator is still attached.

I had the same problems with the older "latest O2 diff" and then built a
kernel to disable the Adaptec controller and add support for a Symbios
based scsi controller which I plugged into the PCI slot.
I got the same errors from that controller.
