I believe the way these were made in the past is using the `vnd(4)' driver;
you simply disklabel it appropriately (with a volume header and an 4.2BSD
partition for the data), init the volume header and populate it with the
bootloader(s), then newfs and copy the install sets to the FFS (aka 4.2BSD)
partition.

Once that's done, you burn the resulting image to CD; though I believe this
all requires drives that can do 512-byte-sector access (cf. Sun CD-ROMs).

For an older bootable image, check out the following URL, but note that the
code is based on much older sources, so I don't recommend it as an install
method (just as an example of how this can/is supposed to work).
