Sounds like your root filesystem to totally hosed.   I Had a similar
problem when developing the WD33C93 SCSI driver, but got out of jail by
falling back to a net-boot root filesystem.

The critical path to get back on air is to reinstall /sbin/init and
rebuilding /dev.  Better yet - newfs and start over!
