It's a PMAG-D (the TURBOchannel designation is PMAG-DA).

In NetBSD-current, although I don't think there's a recent binary
snapshot. 1.5 when released will support your card. There exists an
unsupported patchkit against the 1.4 kernel source, although it seems
nobody has tested it yet.
