ISA IO panic

(Sorry for my late replies about this.. I wasn't subscribed to port-ofppc.

I was able to test some of the mapping routines, especially the ones for PCI,
on macppc.  However, I had no way of testing the ISA ones, as those require an
actual machine with ISA.  I'll take another look over that section of code and
see what I can do there.

Basically, the problem up to this point is, that it needs the ISA maps to talk
to the serial ports, and then wire up the console.  So this is the early stuff
that basically has to work to get to where you can debug.
