I've seen this, too. It's a simple fix, although it might not be a fix 
that everyone likes. Plug the internal SCSI cable into the 5 MB/sec SCSI 
connector (the one farther from the edge of the motherboard) and change 
all of your OF references from scsi-int to scsi. My guess is that OF has 
timing issues with the synchronous bus.

I've had to do this on a number of machines, so on my last machine, I just 
found an ATTO card that I could boot from directly.
