The short answer would probably be:
Put both, the kernel and the loader into the CD's root directory and
burn it as iso9660. All OF versions used by apple can read that.

Yeah, it's odd that older OF versions can't read HFS but do support
MSDOS and iso9660 filesystems. The commands used on OF1 and OF3 are
more or less the same, only the device names differ and OF3 has more
useful devaliases.
