I'm seeing the same thing on my Power Mac 6500.

X reports the "Primary Device" as 12. The ATI driver reports the card it 
finds (and later doesn't find) as 18. While 0x12 == 18, this 
inconsistent reporting raises my suspicions. Is anyone using the ATI 
driver on a lower-numbered PCI device?
