Yes, I downloaded the patch for System Disk.  I tried with and without

No, I have not.  I will try it.  However, I get the same result with or
without the video patch.

I do have the kbd plugged into the monitor.  I will try a direct
connection to the box.  However, I can input commands with the keyboard
(kbd) with no trouble.  I can input through kbd and read output on the
terminal through ttya.

I seem to recall booting into OF once and getting a display some few
years ago.  
I wonder if there is some obscure setting.
