fredb> I have a similiar problem with this one:

This is a cause that some initializations don't execute inside
NetBSD.

Does this CS slot card work fine in MacOS? If initializations
is completed inside MacOS, it will work fine in NetBSD.

# CS slot ether card in my LC630 works fine now, some japanese
# users report it works with installing extend function driver
# or script.(I don't know that kinou-kakuchou in japanese is
# called in english.)
