It is a 3com 40-0196, I think that it was also used in 3com509 series
nics. Dmesg on a 1.5.2 system running on a Quadra 700 says,
nubus0 slot e: Fast EtherTX NuBus (Vendor: Farallon Computer, Part: PN-990)=
 Type: 0004
0001 0108 0101 not configured.
