I've experienced an identical error after installing 1.4.1 on a IIvx,
20 mb ram and a conner cd340s hard drive - 40 meg swap 27omb root/usr
and boooting off an apple/quantum 80mb drive with a minimal install of
7.5.5. I have checked the termination and it is correct.

I have a feeling that 1.4.1 has broken support for the IIvx, as I gave
up in disgust and installed openBSD 2.6 which runs flawlessly on the
same configuration.
