You aren't going to believe this, but I

1) Low-level reformatted the drive, checking & mapping out bad blocks
2) Reformatted it again, using Apple's driver this time, instead of APS 4.1
3) Used Apple's Setup to create the partitions
4) Used Mkfs to set the partition types, and create file systems on them
5) Reinstalled the kernel and base

and the results are EXACTLY the same! I am beginning to think that the
kernel is hosed. I will get a new copy.
