If I read the debug output above, you've got 4 1GB partitions, only one
of which is a NetBSD partition(the root one).  The root partition appears
to be the third physical partition and is located above the halfway point
of the drive(beyond the 2GB point).  The installer has been somewhat 
notoriously flaky in regards to handling root partitions that are large
and located further into the drive.  Your best bet would be to somehow
move the root to the first physical partition and to make it less than
1GB in size(500MB or less is my recommendation).  I haven't really had
a chance to figure out why things are so flaky.  And, I'm not likely to
figure it out soon as I'm fairly busy right now(I just switched companies). 
But, try that and let me know if it works better.  If it does, it'll
give me(or whoever eventually looks at it) a better clue of what to look
for and fix.

p.s.  If you do shuffle the partitions around, I'd recommend that you
	create a "swap" partition while you have a chance.  Even with
	52MB of ram in my system, I still have some swapping/paging
	happen when I try to run something like mozilla ;-)...
