In all likelihood, here's your problem.  What kernel are you using?  The
1.2.1 distribution kernel?  If so, that kernel explicitly doesn't support
any of the '040-based Mac's.  There is no SCSI driver for these machines
in it.  You will need to upgrade everything to -current and try that
instead.  You might have trouble with the accelerator, but I'm not sure.
Please let me know if it does work, and I'll add it to the list of working
accelerators.
