Here's my fstab:
/dev/sd1a       /       ufs     rw 1 1
/dev/sd1b       none    swap    sw 0 0
kern            /kern   kernfs  rw 0 0
proc            /proc   procfs  rw 0 0

 Looks okay to me.  Is anything out of place?

In my last message, I asked about fsck checking the swap partition, but now
I see that it has a zero value at the last place.  So my question is
answered.
