I was using one of Taras' pre-1.2 nubus color kernels (with 1.2 binaries,
wasn't that fun!) and decided to try the lkm.  I did the following:

- Installed "netbsd12" from ftp.netbsd.org in /netbsd
- Made the directory /usr/lkm
- Copied the precompiled video_lkm_combined.o into /usr/lkm
- Added the line "modload -o /tmp/video -e video_lkm_cmd video_lkm_combined.o"
  to my /etc/rc.local (at the end)
- Went into /dev and did a ./MAKEDEV lkm (I didn't have a /dev/lkm)
- Rebooted.

Modload then proceeds to tell me:
modload: /dev/lkm: Device not configured

So, in 50 words or less, what am I missing here?
