I managed to get Cuda almost working this weekend on my Quadra 630. It
turns out that the 630 needs 0x50f00000 needs the I/O map kludge
that was used for the II series machines in macrom.c. I also ported
most of the Cuda and some of the ADB source from MkLinux. 

There's still quite a few bugs to work out, and scsi96 still isn't
working, but at least we're making progress.
