I've seen this exact same problem, and was told that cu expected CD to 
be on in the modem.  This wasn't an option for me.  Instead, lately I've 
been using a fairly kludged setup in which I use dip to dial the phone 
(because cu can't do that) and slattach/ifconfig/route to set up the 
SLIP connection on my end (because I haven't been able to get dip to 
work beyond dialing the phone).
