I think this is the most likely, based on your symptoms.  What are you
using for formatting software?

The new scsi driver is a problem (that's been fixed), but upgrading your
binary set won't change the scsi driver.  It shouldn't affect the fsck,
either.  That's not changed from 1.1_ALPHA to 1.1.
