In case someone cares, I noticed that using the BSD/OS version and
writing the audio to stdout and piping it to mpg123 works, the audio may
be desynched by a fraction of a second, but short of that its okay. Like
this:
mtvp -a2 my.mpg | mpg123 -b 1024 -

Without buffer the result is quite horrible.
