To which a number of people pointed out that I should be using minor device
number 3.

Arrrg!  I knew there was an off by one relationship somewhere with pc com
ports, but it's actually that COM1: <-> com0 -> tty00, COM2: <-> com1 -> tty01,
etc and not com1 -> tty00, etc.  If I'd thought about it for a few seconds I
would have realised that, but I was hung up on it being a PCMCIA problem :-(

Thanks to those who put me right.  Both my ethernet and modem cards now work
properly!
