The "Where'd the card go?" message is also symptomatic of the IO port
mapping lossage I described earlier, which you can try avoiding with
config file lines like:

options PCIC_ISA_ALLOC_IOBASE=0x380	# 0x400 doesn't work--address lines?
options PCIC_ISA_ALLOC_IOSIZE=0x80
