You can hardcode a path to a kernel into the kloader options, but it's
not clear to me how or if this fits into an automated dhcp/nfs
bootstrapping procedure.

I suppose one could try hardcoding a path into the kloader options and
give it a try... So long as the kernel knows it's going to get it's
NFS from root, which isn't too difficult (with the config option). The
new kernel (on the NFS root) would also have to know the same thing I
suppose, and a regular netboot should work from there it seems.

Can someone confirm if this works on Dreamcast?

The IP upload slave method was sort of clumsy, but it's known to work,
and the CD that you boot on the Dreamcast is very simple and won't go
out of date. Unlike other stuff.
