X11 comes as a collection of half a dozen optional "sets" during
initial system install.  (I.e., you don't use pkgin/pkgsrc/pkg_add
to add X11.)

The X sets are described in the INSTALL documentation.  I suggest
another read of:

If you forgot to select these sets during initial installation you
can still add them later.  Run sysinst(1) for that.

(For a long time, sysinst used to be installed only installation
media but at least with NetBSD-9 upwards it's also runnable from
an installed system.)
