I've got a Mylex 1164DP installed in a 164LX running 3.1. Surprisingly, SRM detects 
and happily boots from the controller.

However, it's pretty easy to get the system to crash with this card installed;
simply newfs'ing a large enough partition will do it. No panic, just a lot of
errors are spewed to console and then any disks attached to the card are unusable.


What I get looks like this:

ld1: I/O error - software timeout (0xdead)
ld1b: error writing fsbn 192527520 of 192527520 - 192527551 (ld1 bn 244956320; cn)


I've also noticed this line during boot, well before any trouble happens:

mlx0: unknown firmware init error 78:36:00


I can't rule out hardware failure, but the card works fine in the i386 box I
configured it in.

Any pointers as to where to start?
