Is your CDROM drive set to 512 byte blocks? (Default is 2048.) Usually
there is a jumper on the CDROM drive, somteimes labeled "UNIX mode".
Maybe the firmware doesn't like your CDROM drive, try an other one.
Original DEC (obviously), Plextor and old Toshiba 34xx drive work fine.
