I have answered part of my own question.  The AlphaPC 164 machine I tested
with IDE is different enough from the PC164LX machines I have in that the
PC164 can't see the IDE disk device from the SRM.  It does seem to see the
controller, however.

At any rate, when I moved the IDE disk that I set up on the PC164 to a
164LX box, I can do a showdev that shows the drives manufacturer, and
doing boot from that device appears to work as well as it needs to (need
to build a kernel with root on wd0a).

I'd still like advice on making an IDE bootable disk on a PC164
motherboard, but at least I'm not in as much trouble as I thought I was.
