update to 0.9.0

  stacktrace.c + stacktrace.h = libstacktrace with API
  compatible with GLIBC's backtrace(3).
  See install.stacktrace Makefile's target.
  Tested under NetBSD, FreeBSD, Solaris and Linux.

  Code clean-ups
