Update to libXfixes-4.0.4.

4.0.4:
Add README with pointers to mailing list, bugzilla & git repos
Migrate to xorg macros 1.3 & XORG_DEFAULT_OPTIONS
Replace static ChangeLog with dist-hook to generate from git log
nuke RCS Ids
Janitor: Correct make distcheck and gcc/sparse warnings.
Rename parameters to clarify QueryVersion/QueryExtension.
