pkgsrc doesn't support installing extra files into /usr/pkg that aren't
managed by pkgsrc tools, so running h2ph in this way isn't supported.
However, you can accomplish what you want by first installing
devel/p5-perl-headers, which will install the .ph versions of /usr/include
headers, and as a side-effect will create the correct directories
needed by h2ph.
