Solved it ;)

I noticed that the responsible inode was always the last one in a
cylinder group, and some playing with ddb and breakpoints finally gave
the clue. The fix was in Revision 1.33 of
src/sys/ufs/ext2fs/ext2fs_alloc.c, which has been pulled up to the
NetBSD-4 branch, but not to NetBSD-3 :-(
