Have you tried using /dev/cd0a? For i386 and AMD64, as far as i can
recall, /dev/cd0d doesn't work as "d" being the entire disk. I just
thought of using "a" then since thats the way to mount using floppy. By
using /dev/cd0a it works for me and never fails :).
