I have personally used:
Qualcom CDMA MSM (found in Kyocera KPC650 EVDO interface)
Sierra Aircard 580
with basically ok results.

Aircard 580 with Sprint. Were I starting fresh, I'd get a KPC650 with
Verizon service.

The big bugs with these cards, and likely almost all
ohci/uhub/ugensa/ucom cards are:
removing the card panics the system; use drvctl to detach first
After losing a connection, sometimes a retry doesn't work cleanly.
In practice these are not too annoying.
