Yes, I have.  I am running a fairly recent -current on a Fujitsu
lifebook S7020 which also requires the azalia driver for sound.

Unfortunately, I cannot help much as all it took for me was finding
the right mixer setting to tweak and the CD audio came through fine.
I noted in your mixerctl output you had all the mixers set to their
maximum so I don't think is your problem.
