I have had same problems. Over the past few years, I have tried a few
different implementations (like Earl Hood's and Richard Verhoeven's,
Wolfram Schneider's and Bob Beck's) with bad luck (probably because the
varying range of man and mandoc syntax).

Try groff.

groff -mandoc -Thtml

p.s. Anyone want to help me with making cawf and its definitions behave
good enough to replace groff for just formatting manpages?
