I had several kinds of trouble using the block devices for this. I now always
use /dev/rcd?[acd] for reading a disc. So the line looks like this:

dd if=/dev/rcd2a of=img bs=2048

Never had trouble with this. But i never tried how this works with mounted
CDs, which was the original question AFAIK.
