I have the following in my /etc/rc.local:

    mixerctl -w outputs.master=111,111
    mixerctl -w outputs.master.mute=off
    mixerctl -w inputs.cd.mute=off

I needed to set the outputs.master explicitly as my card was
creating noise at maximum.
