I can't get an SMC Ultra Elite to function in an i386; the kernel
faults as soon as it's done with autoconf.  This is 1.0.

There is an NE2000-alike card in the box, as well.

For example:

ed0 at isa0 port 0x240-0x25f irq 12: address 00:40:95:34:22:57, type NE2000 (16-bit)
ed1 at isa0 port 0x300-0x31f iomem 0xcc00-0x10bff irq 10: address 00:00:c0:7a:9c:87, type SMC8216/SMC8216C (16-bit) aui
npx0 at isa0 port 0xf0-0xff: using emulator
biomask 4040 netmask 141a ttymask 1a
vm_fault(f81a0ba4, a2280000, 3, 0) -> 5
kernel: page fault trap, code=0

Has anyone gotten this configuration to work before?  It works fine with
just the NE2000, or with the NE2000 and another type of card.
