I think you should try making an /etc/ppp/pap-secrets file. I'm not sure
if I'm interpreting the ConfRej and ConfReq stuff correctly, but it looks
like your machine keeps refusing to do PAP, and the cisco keeps asking for
it.

I do know that if I rename my pap-secrets file, the pppd negotiation never
gets past LCP.
