I'm wondering if we should use that mode for man-refs under
<title> or if we can do that automagically ? The problem is that
man-refs are entities and we shouldn't be adding entities like
&man.make.1.plain; -- this is bad.

I would go for the automagically way but I have no clue on how
to implment it, although I'm trying to figure out..
