Scheduled resync with AnonCVS.NetBSD.org repository.

Update htdocs/guide. Major changes are:
  - XML markup policy (normalization and cleanup).
  - Added various XML ID attributes to avoid extra commits.
  - Various content improvements in chap-{boot,cgd,cvs,cons,rc}.xml.
  - chap-rmmedia.xml: Add more information about handling ISO images.
