 Had it happen again. this time I was doing something that could be
 reproducing it. I was testing kernel changes in qemu. with the workflow:
 
 1. build kernel
 2. mount the vnd-image
 3. mv kernel to vnd-image
 4. umount vnd-image
 5. boot qemu to read/write said vnd-image
 
 it failed between step #4 and #5, so maybe it was not invalidated on all
 cpus yet(??). It's a pretty big file, 4GB.
 caveat: my vnd filesystem was not clean
