 I have been fighting for the CD-ROM problem for quite some time now.
 The good news is that my installation problem is caused by a broken
 optical drive;
 the bad news is that I can no longer replicate the problem without
 that drive (definitely dead now).

 The broken optical drive is the superdrive that came with Mac mini G4,
 and I don't have
 any spare drive that can fit the 50-pins PATA interface. I did try to
 workaround this issue:
 (1). Using a USB external optical drive, successfully boot into
 installer from CD-ROM, but
     failed to mount that CD-ROM in installer or shell. The uhidv is
 not configured for the drive.
 (2). Copy the iso image into /tmp directory, but fail to mount the iso.

 Any suggestion? Is it possible to simulate the installation process in
 a working NetBSD environment?
