rev. 1.159 of usb_subr.c (on Jun 22 2008, before the NetBSD-5 branch)
changed the order so that the address is now assigned after the
initial descriptor fetch, so the bug you describe was fixed as a side
effect.
wrt the 64 byte read: I've just changed -current to do so which made
some strange embedded device work. Didn't see a necessity for the
additional reset afterwards, but that would be the first thing to
try if a device shows up which still doesn't work.
